The Catholic Diocese of Raleigh, in consultation with experts in Catholic bioethics and law, has created an Advance Directive Document (available in English and Spanish below). We offer you this document, which is designed for the state of North Carolina, to assist you in ensuring that your care and treatment is consistent with the Catholic faith and your wishes.
